Here is a chart you can use while monitoring your students on Days 1-3 of the Tabor model, to plan for reteach or extension groups on Thursday & Friday.

Teachers will write student names in the boxes in the form as they notice students struggling or succeeding with the math concept focused on that week. I usually make notes about the specific skills the child is struggling with. For example, if the week's focus is fractions, and a child is struggling with the keeping the whole or total as the denominator, I would write their name, with "whole" or "denominator" below their name. Then, I'll use this form to plan my small groups for Thursday and Friday. I can plan an extension lesson for students in the high column.
